Polycystic ovary syndrome (PCOS)

What is PCOS?

It is a hormonal disorder common among women of reproductive age. It is a condition that affects a woman’s hormone levels.

Women with PCOS produce higher-than-normal amounts of male hormones. This hormone imbalance causes their body to skip menstrual periods and can lead to female infertility.

If you have PCOS, your ovaries produce unusually high levels of hormones called androgens. This causes your reproductive hormones to become imbalanced. 

Small cysts may develop on your ovaries (fluid-filled sacs) due to lack of ovulation (anovulation).


Causes

The exact cause of PCOS is unknown. Many women with PCOS have insulin resistance. This means the body can't use insulin well. Insulin levels build up in the body and may cause higher androgen levels. Obesity can also increase insulin levels and make PCOS symptoms worse.

PCOS may also run-in families.

Low-grade inflammation: People with PCOS tend to have chronic low-grade inflammation. Your healthcare provider can perform blood tests that measure levels of C-reactive protein (CRP) and white blood cells, which can indicate the level of inflammation in your body. 


Risk factors

PCOS in adults includes type 1 diabetes, type 2 diabetes, and gestational diabetes. Insulin resistance affects 50%–70% of women with PCOS leading to a number of comorbidities including metabolic syndrome, hypertension, dyslipidaemia, glucose intolerance, and diabetes. 


Symptoms

The symptoms of PCOS may include:

  • Missed periods, irregular periods, or very light periods

  • Ovaries that are large or have many cysts

  • Excess body hair, including the chest, stomach, and back (hirsutism)

  • Weight gain, especially around the belly (abdomen)

  • Acne or oily skin

  • Male-pattern baldness or thinning hair

  • Infertility 

  • Small pieces of excess skin on the neck or armpits (skin tags)

  • Dark or thick skin patches on the back of the neck, in the armpits, and under the breasts
